The Director is responsible for creating an atmosphere that provides for continuity care and quality of life for all sisters. This is accomplished by providing clear direction and expectations to Leadership and all others involved. General Responsibilities: Relate to Regional Coordinators and individual Sisters regarding their physical and/or mental health assessments; recommend doctors, accompany Sisters to doctors when appropriate. Ensure that the medical, psychosocial, emotional, and spiritual needs of Sisters are addressed and followed up on the Care Plan in an appropriate and supportive manner.

Provide a quality work environment through teaming principles, effective leadership, and a demonstrated positive attitude. Interact courteously and appropriately with Sisters, family members, visitors, and other employees under a variety of conditions and circumstances. In collaboration with health care team, visit Sisters when hospitalized for serious illness or surgery: With Sisters’ permission, consult with doctors Determine need for special nursing care Assess needs at discharge and arrange for convalescent and follow up care as needed Be present before and after surgery to speak with the doctor/surgeon Ensure the completion of Sisters of Charity Health Care Assessment Form: when Sisters request transfer to a Sisters of Charity retirement facility when requested by a Regional Coordinator Oversee schedule and accompany Sisters to doctor appointments, when needed.

Ensure proper coordination of transportation needs of Sisters to Doctors and hospital appointments, if medically necessary. Contact appropriate Regional about significant health changes with Sisters. Skills and Qualifications: Graduate of an accredited school of Nursing.

Current license as a Registered Nurse in the State of New York, and maintain mandated continuing education requirements. 5 years’ experience in a long-term elder care and 3 years management experience.
